Fat Transfer Under Eyes for Dark Circles and Tear Troughs

Why the under-eye area is different

The skin around the eyes is thin, mobile and supported by very fine ligaments. For these reasons, under-eye fat transfer is not a standard facial fat transfer; it is a specialised application of microfat and nanofat techniques.

Microfat and nanofat for the tear trough

Microfat

Small fat parcels that can be placed in the deeper tear trough layer to restore a smooth transition between the lower eyelid and the cheek. It adds gentle volume and can improve the contour that causes shadowing.

Nanofat

Emulsified fat that is rich in regenerative cells. It is used very superficially to improve skin quality, thickness and fine crepey texture. Nanofat adds little volume, so it is often combined with microfat when both contour and skin quality need attention.

What happens during the procedure

Fat is gently removed from a donor area, usually the abdomen or thighs, using a small liposuction cannula. The fat is then washed and processed to separate the microfat and nanofat fractions. Dr Vahe Karimyan injects tiny amounts in fine layers beneath the tear trough, building contour gradually to avoid over-correction. Recovery specific to the eye area

Swelling around the eyes can be more noticeable than elsewhere on the face because the skin is thin and the area retains fluid easily. Most patients find that bruising and puffiness peak in the first 2 to 3 days and then settle steadily. A gentle cold compress, head elevation and avoiding strenuous activity help during the first week. Many people feel comfortable returning to office work within 5 to 7 days, although residual swelling may remain for a few weeks.

  • Sleep with your head raised for the first week.
  • Avoid rubbing or pressing the under-eye area.
  • Skip heavy exercise and saunas until cleared.
  • Do not smoke, as nicotine reduces fat graft survival.

Longevity compared with dermal fillers

Hyaluronic acid fillers under the eyes typically last 6 to 12 months before the body absorbs them. Fat transfer works differently: a portion of the grafted fat is reabsorbed in the first 2 to 3 months, and the fat that establishes a blood supply can remain for many years. Some patients find the result lasts longer than repeated top-ups, while others choose a small refinement later. Individual outcomes vary.

Who is a good candidate

Under-eye fat transfer tends to suit patients with tear trough hollowing, a tired or sunken appearance, or thin crepey skin who want a natural, longer-lasting result. It may not be suitable if you have significant pigmentation, active eye conditions, very poor skin elasticity, or medical conditions that affect healing. Suitability is confirmed only at a face-to-face consultation.
